Patient Paper Work
Dear Patients, Welcome to Associates in Internal Medicine, P.C.
. Because we know your time is valuable, we have posted the
required New Patient Paper Work on our website for your
convenience. Please download, read and bring the completed and
signed papers with you when you come in for your visit. Some of
this paperwork is government mandated. We apologize that it is
so lengthy. All of the required paper work will be scanned into
your chart for future reference. Office policies must be signed
in order to establish yourself as a patient with our group. If
for some reason pages to not print clearly, please do not fill
them out. We will supply a clear copy to you when you come into
the office. Please be advised that if we accept your insurance,
you will be required to present a copy of your insurance card
every time you visit the office.
